Rooted in Oregon is committed to providing accessible and comprehensive mental health support through upcoming clinical services and holistic care models designed to provide specialized support where it is needed most.
Medication Management
Rooted in Oregon is expanding to include medication management with our PMHNP, Natasha Avalon-Gardner. Services will include psychiatric medications, treatment for substance use (including MAT), and experience supporting clients managing HIV medications. This will allow us to extend medication support to rural and underserved communities, where access is often limited.
Case Management
Our upcoming Case Management services are designed to bridge the gap between clinical care and daily stability. We provide dedicated support to help you navigate complex systems, coordinate with other care providers, and connect with community resources that support your overall well-being. This collaborative approach ensures that your path to healing is grounded in practical, sustainable solutions that meet the unique needs of our rural Oregon communities.
